RN Iohn Phylip sick ever sinc thursday being the 30 of Decēb \novemb/ sent to me agayne the 4 of Decemb sick very greevously in his hart & body he is swelled in his body from the belly vnto his hart. he is Costive & so hath cōtinued ever sinc thursday last & is very exceeding ill & weeke. GJ M^r Mathew Claver of Foskett. sent his water w^th a letter April. 6. ☿ ho. 10. 6 a m. 1603 Wherin he Complayneth of a trembling hastynes and fulnes of his head & sences his body full of wynde, a great noyse in his left eare many tymes troubled w^th a stitche in the small of his backe. RN Margaret Tarelton \Thorne/ the wife of Will Tarelton a tealer \of Newport./ 40 yeres old 16 yeres maried sick & hath had her disease growing vppon her this 12 monets {sic} & better quærit novemb. it taketh her every foote. Novemb. 13 die ☾. h. 12. 6. mi 1598. shee was so weak shee could not come but sent her servant mead it ariseth from her hart vpward to her throot ready to stop her winde

RN Raynold Davyes of the glover of Stonystratford sent his boy w^th his water keepeth his bed most of all but sometimes riseth vppon the 11 of novemb^r it took him first, but was very ill handled w^th it vppon die ☉ sent his boy to me for my consell this 16 of novemb die ♃ h. 8. ant m. 1598 he thincketh he took it w^th a bruise he took w^th the caring of hides a yere since. 40 yeres old.

RN M^r Powell of London sent to M^r Cooke to have some thing to thicken his seed troubled w^th the gonorrea lyeth on his bed back els cānot sleepe is to may {sic} shortly wanteth virility & erection sweateth at night. Ian. 11 h. 8. 20 ant m 1623¹

RN Goodmā|dy| Nueman of Brodwell. of 45 yeres of age found mutch ease by y^t w^c phisick w^c was given her & wrought vpward & downeward w^th her & shee was very prety well after it for 2 or 3 days. &|b|ut st the thirde day after it tooke her agayne & she is mutch troubled w^th the winde cholick & w^th a hot & drye stomacke. & sent her daugh{t}{er} agayne to me. she layd to it a litle wax & butter & now & then a poultis of milke & a weddensday last was senet it broke about the 12 of Ap desyrous to have my counsell the 20^th of Apr h. 5. 30 pm 1620 It is broken & did run & voyd filthy \yellow/ stuffe

RN Alce Crowly troubled w^th the ague ever sinc Layten feyre. about 40. every third day keepeth her bed & shaketh sent for a remedye. the 19 of feb. die ☾ h. 11. 1599.
